Job summary

Amedisys is seeking an LPN for full-time home health visits in Savannah, GA. You will verify plans of care with RNs, collaborate with physicians, teach self-care, monitor vital signs, and reconcile medications during each visit.

The role emphasizes accurate documentation, aseptic technique, and timely communication to ensure high-quality patient care in the home setting.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify as patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform procedures ordered by physician (e.g., foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
