Job summary

Amedisys is seeking an LPN for full-time home health visits in Georgia. The role involves delivering patient-focused nursing care in the home, coordinating with RNs and physicians, and ensuring care plans are followed accurately.

Responsibilities include vital signs, med reconciliation, patient teaching, and timely documentation. Eligibility requires an active LPN license in the state, CPR certification, and a valid driver’s license with reliable transportation.

Qualifications

• 1 year of clinical LPN experience.
• Current LPN license for assigned state(s).
• Current CPR certification.
• Valid driver’s license and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform physician-ordered procedures (e.g. Foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
