Job summary

Amedisys in Florence, SC is hiring a Licensed Practical Nurse for full-time day shifts to provide in-home patient care. Verify plans of care with RNs, collaborate with physicians, teach self-care, monitor vitals, review medications, perform procedures, and document accurately.

Responsibilities

• Verify plan of care with the RN prior to each home visit.

• Review medications and perform reconciliation.
• Prepare equipment for treatments using aseptic technique.
• Complete timely and accurate documentation.

Responsibilities

• Verify plan of care with the RN prior to each visit.

• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
