Job summary

Amedisys is seeking a PRN Home Health LPN to join our team in a dedicated homecare setting. Verify plans of care with the RN, support patients and families, and collaborate with physicians to meet evolving needs in patients' homes.

The role offers flexible PRN shifts, competitive hourly pay, and a comprehensive benefits package including medical plans, PTO, wellness rewards, mental health support, retirement options, and family services. Being on the move in the community is often required.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify them if patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
