Job summary

Amedisys is seeking a PRN Licensed Practical Nurse to support in-home patient care. You will verify each plan of care with the supervising RN prior to visits and collaborate with physicians and RNs to ensure patient needs are met while delivering compassionate, in-home care.

You will teach patients and caregivers self-care techniques, monitor vital signs, review medications, and prepare equipment for treatments. Flexibility and a patient-centered approach are valued.

Qualifications

• One year of clinical LPN experience.
• Current LPN license in the applicable state(s).
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
