Job summary

Amedisys is seeking a licensed practical nurse (LPN) to provide home health and hospice care in Georgia. The role emphasizes patient-centered care, collaboration with RNs and physicians, and hands-on clinical duties during visits.

Qualifications

• One year of clinical LPN experience is required.
• Current LPN license valid in the state(s) you work in.
• Current CPR certification is required.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to address patient needs and notify changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform procedures ordered by physician (e.g., Foley catheter change).
• Prepare equipment and materials using aseptic technique.
• Complete documentation timely and accurately.
