Job summary

Amedisys, one of the largest home health and hospice providers in the U.S., is seeking a part-time Licensed Practical Nurse to support patients at home. You will verify care plans with the RN, collaborate with the care team, and teach patients self-care techniques while monitoring vital signs and medications.

Qualifications

• One year of clinical LPN experience required.
• Current LPN license for the assigned state.
• CPR certification required.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs.
• Teach patients and caregivers self-care techniques.
• Check vital signs during visits.
• Review medications and perform reconciliation at each visit.
• Perform physician-ordered procedures (e.g., Foley catheter changes).
• Prepare equipment using aseptic technique.
• Document accurately and timely.

• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician ( i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
