Job summary

Amedisys is seeking a licensed practical nurse to provide hands-on care to geriatric patients in the home. You will verify care plans with the RN, coordinate with physicians, and support patients and families with essential self-care techniques.

The role is full-time with paid training, mileage reimbursement, and a comprehensive benefits package including medical plans, PTO, 401(k) with company match, and wellness resources. You will travel to patient homes as part of routine duties.

Qualifications

• One year of clinical LPN experience.
• Current CPR certification.
• Current LPN license, active in the state you work.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs and report changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs at visits.
• Review medications and perform medication reconciliation.
• Perform procedures ordered by physician (e.g., Foley catheter changes).
• Prepare equipment for treatments using aseptic technique.
• Complete timely and accurate documentation.

• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
