Qualifications

• One year of clinical LPN experience.

• Valid driver's license and reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to address patient needs; notify changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at visits.
• Perform physician-ordered procedures (e.g., Foley changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.

• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
