Job summary

Amedisys is seeking a licensed practical nurse to provide in-home care in Conway, South Carolina. This full-time day role offers the flexibility and support of a leading home health provider, with patient-focused service and comprehensive benefits.

You will verify care plans, collaborate with physicians and RNs, educate patients and caregivers, monitor vitals, reconcile medications, and perform procedure-based visits while maintaining thorough documentation.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met and notify changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
