Job summary

Henry County Health and Rehabilitation is seeking an LPN to join our care team in Paris, TN, working the night shift from 7 PM to 7 AM. A $2,500 sign-on bonus is offered to eligible applicants, with comprehensive benefits and a supportive team environment.

You will supervise CNAs, participate in admissions and transfers, assess residents, administer medications, and maintain care plans. This role emphasizes resident safety, confidentiality, and collaboration with leadership to ensure quality

Qualifications

• Current unrestricted LPN License for the state of Tennessee.
• Current CPR/BLS Certification.
• Night shift availability.

Responsibilities

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess resident’s conditions, prepare, and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report any changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
• Perform other assigned duties given by clinical or administrative leadership.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.
