The Grand Rehabilitation and Nursing at Queens is seeking an experienced LPN to join our skilled nursing facility team in New York. We offer full-time and part-time positions with a pay rate up to $34.48 per hour and daily pay options.

The ideal candidate holds a current NY LPN license, is passionate about geriatric care, and can work independently while supervising CNAs and coordinating resident care across units.

Qualifications

• Current LPN license in NY is required.
• Experience with geriatric population preferred.
• Ability to work independently and supervise CNAs is a plus.

Responsibilities

• Provide medication administration and IVs where applicable.
• Take and record vital signs and monitor patient outcomes.
• Deliver resident care and family education.
• Communicate with physicians, NPs, and other health professionals.
• Supervise CNAs and coordinate care in SNF/Long Term Care units.

LPN (Licensed Practical Nurse) Job Description:

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
