Job summary

The Grand Rehabilitation and Nursing at Queens is seeking a dedicated LPN for full-time roles in our Skilled Nursing and Long Term Care units. The ideal candidate will hold a NY LPN license, provide comprehensive nursing care, administer medications, and monitor patient responses to treatments.

This position requires independence in evaluating residents, effective communication with care teams and families, and a commitment to high-quality geriatric care.

Qualifications

• Current NY LPN license is required.
• Ability to evaluate and treat residents independently.
• Experience in geriatric or long-term care settings is preferred.

Responsibilities

• Provide nursing care to residents in SNF/Long Term Care units.
• Administer medications, including IVs as trained.
• Take and record vital signs and monitor treatment outcomes.
• Educate residents and families about care plans.
• Communicate with physicians, NPs, families, and health professionals.
• Supervise CNAs and coordinate with care teams.

Looking for a full time talented and motivated Licensed Practical Nurse (LPN) to join our dynamic team. The position is within a Skilled Nursing facility. Candidate should possess passion for geriatric population, must contain a license in the state of NY, be able to evaluate and treat independently without direct supervision, and posses the desire to perform functional therapeutic activities to assist residents improve in their experience. Hiring Immediately.

Rate: Up to $34.48 an hour

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs
