Job summary

Patriot At Home in Dayton, OH is seeking an experienced Registered Nurse for a full-time home health role. You will coordinate total patient care, evaluate nursing service effectiveness, and handle admissions, transfers, recertifications, resumption of care, and discharges in patients' homes.

The position offers a competitive salary with the option to earn extra by picking up visits as per diem, mileage reimbursement, and comprehensive insurance.

Qualifications

• Graduate from an accredited School of Nursing.
• Licensed in the state as a Registered Nurse.
• One or more years of experience in community/home health agency or hospital preferred.
• Knowledge of Medicare and Medicaid guidelines.
• Criminal background check.
• Must have a current CPR (BLS) certification.

Responsibilities

• Have coordination of total patient care.
• Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis.
• Perform admission, transfer, recertification, resumption of care, and discharge Oasis for the home care patient.
• All other duties as assigned.
