The Phoenix Rehabilitation and Nursing Center in Elmhurst, NY is seeking a full-time Licensed Practical Nurse (LPN) to join our skilled nursing team. You will provide direct patient care, administer medications, monitor vital signs, and collaborate with physicians and CNAs.

This role offers on-site work with day, evening, and night shifts, a $10,000 sign-on bonus, daily pay option, and benefits including medical, dental, vision, and more.

Qualifications

• Geriatric care experience preferred in SNF settings.
• Ability to perform medication administration and vital signs checks.
• Provide patient education and communicate with care team.

Responsibilities

• Provide care for residents in SNF/Long Term Care units.

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs

Shifts available: Day shift , Evening shift, Night shift
