The Phoenix Rehabilitation and Nursing Center in Harlem is seeking a full-time Licensed Practical Nurse (LPN) to join our geri atric care team. The role focuses on resident care in a skilled nursing facility, with opportunities for independent evaluation and treatment within a supportive team.

• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
