The Phoenix Rehabilitation and Nursing Center is seeking a full-time Licensed Practical Nurse (LPN) to join our skilled nursing team. The position offers a $10,000 sign-on bonus and daily pay options, with shifts available across day, evening, and night.

Qualified candidates must hold a current LPN license and have a passion for geriatric care. Independent assessment, resident care, and collaboration with a multi-disciplinary team are essential to improve residents’ experiences.

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs
