Lutheran Senior Services is seeking a Licensed Practical Nurse to provide and coordinate resident care across physical, psychosocial, and functional needs. You will work with a multidisciplinary team to develop individualized plans and ensure accurate documentation in the EMR.

The role includes med administration, potential IV therapy, and collaboration with nursing leadership to ensure regulatory compliance and high-quality care. LPN licensure and CPR certification are required.

Qualifications

• Licensed Practical Nurse license active in state of practice.
• CPR certification is required.
• Specialized training or experience in gerontological nursing preferred.

Responsibilities

• Practice nursing within licensure scope and follow Nurse Practice Act.
• Collaborate with interdisciplinary team to define individualized care plans.
• Administer medications and monitor IV fluids and treatments.
• Initiate IV therapy if IV-certified.
• Maintain documentation and care plans in EMR per policy.
• Observe resident status and coordinate with services for continuity of care.
• Ensure safety and maintain a clean, functional environment.
• Supervise certified staff to meet EverTrue standards of care.
• Report incidents and support investigations per regulations.
