Lutheran Senior Services in Kentucky is seeking a Licensed Practical Nurse to provide and coordinate resident care across physical, psychosocial, and functional needs. You will work with an interdisciplinary team, administer medications, and ensure regulatory compliance in a supportive geriatric environment.

This role includes IV therapy initiation where certified, EMR documentation, and adherence to EverTrue standards. Strong teamwork and careful observation are essential for quality outcomes.

Qualifications

• LPN licensure in state of practice.
• Must be CPR certified.
• Specialized training or experience in gerontological nursing preferred.

Responsibilities

• Practice nursing within licensure; follow Nurse Practice Act and keep license active.
• Collaborate with interdisciplinary team to define individualized care plans.
• Pass/oversee medications; administer meds and monitor IV therapies when ordered.
• May initiate IV therapy if IV-certified.
• Maintain observation/assessment/documentation and MDS knowledge.
• Utilize EMR for complete clinical documentation per policy.
• Observe resident status and coordinate with services; document reports.
• Maintain safe environment and uphold safety practices.
• Oversee certified staff to ensure EverTrue standards of care.
• Report incidents per state and federal protocols.
