Job summary

Lutheran Senior Services is seeking an LPN to provide nursing care for residents, coordinate services, and work with an interdisciplinary team. The role includes medication administration, IV therapy (if certified), and comprehensive documentation in the EMR.

Qualifications

• LPN licensure in state of practice.
• Must be CPR certified.
• Specialized training or experience in gerontological nursing preferred.

Responsibilities

• Provides nursing care within licensure scope and follows Nurse Practice Act.
• Collaborates with interdisciplinary team to define individualized resident plans.
• Administers medications and monitors IV fluids and treatments as ordered.
• May initiate IV therapy if IV-certified.
• Maintains documentation and uses EMR for clinical notes and care plans.
• Ensures safety and continuity of care across services.
• Oversees staff and ensures EverTrue standards of care.
• Documents incidents and supports investigations per policy and regulations.
• Provides teaching and role modeling to staff for quality care.
• Maintains a safe, clean, and functional environment for residents.
