Qualifications

• Must be a Licensed Practical Nurse (LPN) in good standing and licensed by the state of Oklahoma.
• Minimum 1 year experience in a Skilled Nursing Facility or Long-Term Care environment.
• Diploma from an accredited School of Nursing is required.
• Ability to read, write, and follow oral and written directions at a level necessary to accomplish the job is required.
• Must relate positively and favorably to residents and families and work cooperatively with other associates.
• Must be capable of maintaining regular attendance.
• Excellent communication and teaching skills in both individual and group settings.

Responsibilities

• Provide direct nursing care to residents and supervise CNAs and CMAs.
• Utilize PPE and follow infection control protocols.
• Coordinate with nursing care and resident services to maintain care plans.
• Participate in QA program and government surveys.
• Document diet orders and communicate with dietary staff.
• Chart communications with physicians and medical staff.
• Receive and transcribe physician orders to medical records.
• Assist with discharge planning and update discharge care plans.
• Take and record vital signs and maintain resident charts.
• Administer medications after verifying resident information; report discrepancies.
• Maintain confidentiality and protect residents from neglect or mistreatment.
