Job summary

Southwest LTC is seeking a licensed practical nurse (LPN) for full-time, on-site work in Oklahoma City. You will provide direct nursing care in a skilled nursing facility under supervision, administer medications, monitor health status, and support care plans while ensuring high-quality care in line with policies and regulations.

The role requires an active LPN/LVN license, graduation from an accredited practical nursing program, CPR certification, and preferably experience in long-term care.

Qualifications

• Current LPN/LVN license in the state of employment.
• Graduate of an accredited practical nursing program.
• Current CPR certification required.
• Experience in long-term care or skilled nursing preferred.

Responsibilities

• Provide direct nursing care to residents in accordance with care plans and physician orders.
• Administer medications and treatments as prescribed.
• Monitor residents’ vital signs, conditions, and responses to treatment.
• Report changes in resident condition to the RN or physician.
• Assist with development and implementation of individualized care plans.
• Supervise and support CNAs.
• Maintain accurate and timely medical documentation.
• Ensure compliance with infection control and safety standards.
• Respond to resident needs and emergencies.
• Participate in shift reports and interdisciplinary team meetings.
• Provide compassionate care while maintaining resident dignity and privacy.

The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a skilled nursing facility under the supervision of a Registered Nurse (RN) or Director of Nursing.
