Job summary

Heritage Manor in Oklahoma City is hiring a Licensed Practical Nurse (LPN) for the 3–11 shift, Monday through Friday, full-time. You will provide direct nursing care to residents and supervise CNAs and CMAs to ensure consistent, high-quality care and safe clinical practice.

You will follow the Oklahoma Nurse Practice Act and all federal, state, and local regulations, document care and medications in residents’ charts, collaborate with physicians and families, and support discharge planning while

Qualifications

• Must be a Licensed Practical Nurse (LPN) in good standing and licensed by Oklahoma.
• Minimum 1 year experience in a Skilled Nursing Facility or Long-Term Care.
• Diploma from an accredited School of Nursing is required.
• Ability to read, write, and follow directions as required for the job.
• Able to relate to residents and families and work cooperatively with staff.
• Regular attendance and strong communication skills are essential.
• Excellent ability to teach and communicate with residents and families.

Responsibilities

• Provide direct nursing care to residents and supervise CNAs/CMAs per policy.
• Adhere to infection control protocols and PPE usage.
• Coordinate with nursing care and resident services to maintain care plans.
• Document communications with physicians and families.
• Record vital signs and maintain resident medical records.
• Administer medications following the 5 Rights and report discrepancies.
• Support discharge planning and review discharge care plans.
• Respond promptly to call lights and ensure resident comfort.
• Maintain confidentiality and protect residents from neglect.
