Job summary

Prisma Health is seeking a Nurse Liaison for Home Health Intake to coordinate referrals from hospitals, clinics and home health agencies. You will act as a liaison across care settings and manage patient intake on an ongoing basis.

The role requires nursing education and at least one year in home care, hospice, or post-acute care, with strong communication and documentation skills. Day shifts with some weekend coverage in the Upstate South Carolina service area.

Qualifications

• Education: Graduate of an accredited school of nursing.
• Experience: One year in home care, hospice or post-acute care.
• License/Registration: Valid LPN or RN license recognized by the NCSBN Compact or in-state.
• Driver requirements: If driving is required, valid license and proof of auto insurance.

Responsibilities

• Coordinates internal referrals and intake process for referred patients.
• Acts as liaison between hospitals, medical offices, home health agencies and suppliers.
• Schedules 12-hour shifts with weekend coverage and coordinates patient flow.
• Completes intake accurately and ensures seamless discharge to home care.
• Maintains ongoing communication with case management, physicians, and care team members.
