Duke HomeCare & Hospice in Durham, NC is seeking a Referral Coordinator LPN to serve as a key liaison between hospitals, physician offices, and our home health team. This hybrid role emphasizes EMR management, chart reviews, and accurate order entry to ensure timely care.

Ideal candidates are licensed NC LPNs with 2–4 years in home health, strong communication, and a patient-centered approach. Relocation assistance may be available.

Formación

• Graduation from an accredited Practical Nurse program.
• Current NC LPN licensure (or multi-state license) preferred.
• BLS certification required; will be provided if expired during orientation.
• Working knowledge of Medicare/Medicaid regulations and patient confidentiality.

Responsabilidades

• Review referrals for Home Health eligibility and safety concerns.
• Coordinate Start of Care schedules to ensure timely admissions.
• Obtain updated orders from providers as needed.
• Provide nursing care per physician orders and policies.
• Collaborate with managers and schedulers for timely admissions.
• Communicate with field clinicians about scheduled admissions.
• Maintain up-to-date knowledge of Home Care regulations and Joint Commission standards.
