Job summary

Angels Care Home Health in WaKeeney, KS seeks an LPN Intake Coordinator to facilitate referral intake and coordinate care with the interdisciplinary team and referral sources. You will process referrals using the Home Care Home Base software and ensure timely, compliant intake.

Requirements include an accredited Practical/Vocational Nursing program, active LPN license, valid driver’s license, and at least one year of LPN experience, preferably in home health.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current state LPN license is required.
• Valid driver’s license is required.
• One year of LPN experience, home health preferred.
• Proof of hepatitis profile is required.

Responsibilities

• Facilitate referral intake and coordinate care with the interdisciplinary team and referral sources.
• Process all aspects of referral intake via Home Care Home Base software to ensure timely, accurate processing.
