This role focuses on administrative support while maintaining empathy and patient-centered communication. You will work with families, coordinate with departments, and ensure accurate patient data entry in the EMR system.

Qualifications

• Must be a Licensed Practical Nurse (LPN).
• Strong organizational skills with multitasking ability.
• Excellent written and verbal communication with clients, team, and insurers.
• Attention to detail and accuracy in collecting and documenting client information.
• Proficient in using electronic health record (EHR) systems.
• Ability to maintain confidentiality and handle sensitive information.
• Strong problem-solving and critical thinking skills.
• Comfortable working in a fast-paced, high-pressure environment.
• Strong customer service with compassionate approach.

Responsibilities

• Coordinate activity across departments to ensure correct care at the right time.
• Communicate with patients and families about treatments, meds, and care needs.
• Discuss and consult health history, diagnosis and needs with patients and families.
• Educate patients and families about conditions, medications, and instructions.
• Ensure compliance with facility guidelines and regulatory requirements.
• Maintain complete and accurate patient registration in EMR.
• Establish and maintain relationships with service providers.
• Confidentially manage patient accounts.
• Participate in regular team meetings to address intake process issues.
• Identify areas for improvement in the intake process.
