Job summary

The Lakes Home Care, Inc. is seeking a highly organized LPN to join our Home Health team in a full-time, office-based role.

The LPN will function like an Intake Coordinator, managing patient records, coordinating admissions, and ensuring documentation is complete and compliant. Responsibilities include maintaining charts, tracking missing paperwork, and supporting the clinical team with administrative tasks related to patient care coordination.

Qualifications

• Active LPN license in Florida.
• Experience in Home Health or intake coordination preferred.
• Strong organizational, multitasking, and communication skills.
• Bilingual Spanish and English advantageous.

Responsibilities

• Manage and maintain patient charts and records (electronic and/or physical).
• Ensure all required documentation is complete prior to admission.
• Track and follow up on missing documentation.
• Assist in organizing and maintaining compliance-ready patient files.
• Support clinical team with administrative tasks related to patient care coordination.
