Job summary

The Columbus Organization is hiring a Clinical Intake Specialist-LPN to act as the primary contact for individuals with complex care needs. You will conduct initial visits, perform needs assessments, and coordinate care with the Case Manager to ensure continuity of care.

Role requires travel throughout Dougherty County and surrounding areas with remote administrative work when not on client visits. LPN license and 3+ years nursing experience are required; Medicaid/Medicare knowledge is a plus.

Qualifications

• Requires an associate degree in nursing or LPN certificate/diploma.
• Minimum of three years of nursing experience, preferably in intake/triage or assessment roles.
• Proficiency in EHR systems and strong documentation skills.]
• Knowledge of Medicaid/Medicare and waiver programs is preferred.

Responsibilities

• Conduct initial face-to-face visits to gather health and personal information.
• Perform comprehensive needs assessments of medical, behavioral, nutritional, and social needs.
• Schedule and complete follow-up visits (quarterly, annual, or as-needed).
• Provide options counseling and develop personalized care plans.
• Collaborate with Case Manager to ensure continuity of care and safety.
