Aufgaben

• Answers phones and triages calls to Central Intake Teams.
• Communicates patient and referral information to Central Intake staff and branches.
• Obtains demographic and clinical information to compile referrals.
• Coordinates referral information with Central Intake Nurses.
• Transfers high-tech/nurse referrals to Central Intake Nurses.
• Triage incoming calls and messages from referral sources and relay as needed.
• Gathers information from callers when the patient is not available.
• Provides information about agency services to patients and families.
• Enters referrals into the mainframe as needed.
• Attends trainings and workshops to stay updated on changes.
• Disseminates patient ping data to PCM, clinicians, or sales reps.
• All other duties as assigned.
