Job summary

Angels Care Home Health is seeking an LPN Intake Coordinator to manage the referral intake flow and coordinate care with the interdisciplinary team.

You will process all aspects of referral intake using the Home Care Home Base software, ensuring timely communication with referral sources and accurate documentation.

This Monday–Friday role includes comprehensive benefits, PTO, 401(k) matching, and opportunities for professional growth.

Qualifications

• Graduate of an accredited school of Practical/Vocational Nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• One year experience as an LPN, home health preferred.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.

Responsibilities

• The primary function is to facilitate the intake referral process and coordinate care with the interdisciplinary team and the referral source.
• Will be responsible for processing all aspects of referral intake process via Home Care Home Base software system.
