Job summary

Angels Care Home Health is seeking an LPN Intake Coordinator to facilitate referral intake and coordinate care with an interdisciplinary team. You will manage the referral process and related documentation using Home Care Home Base software.

Qualified candidates have an LPN license, a driving license, and preferably one year of home health experience. The role supports a Monday–Friday schedule with benefits and training opportunities.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as a LPN.
• Current State’s Driver’s License.
• One year experience as an LPN, home health preferred.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.

• The primary function is to facilitate the intake referral process and coordinate care with interdisciplinary team and the referral source.
• Will be responsible for processing all aspects of referral intake process via Home Care Home Base software system.
