Job summary

Angels Care Home Health is seeking an LPN Intake Coordinator to streamline referral intake, coordinate care with the interdisciplinary team, and ensure smooth transitions for patients. You will use Home Care Home Base software to manage referrals and document activities efficiently.

The role requires an LPN license, Koch no; experience in home health is preferred, with Monday–Friday schedule and opportunities for professional development included in the benefits package.

Qualifications

• Qualified LPN with graduation from an accredited practical nursing program.
• Current LPN license in the state.
• One year of LPN experience; home health preferred.
• Hepatitis profile verification completed.

Responsibilities

• Facilitate intake referral process and coordinate care with interdisciplinary team.
• Process referral intake via Home Care Home Base software system.
• Ensure timely, accurate documentation and communication with referral sources.

Education

Graduate of an accredited school of practical/vocational nursing

Current State LPN license

One year experience as an LPN, home health preferred

Current State Driver's License

Job Functions:

The primary function is to facilitate the intake referral process and coordinate care with interdisciplinary team and the referral source. Will be responsible for processing all aspects of referral intake process via Home Care Home Base software system.

Qualifications:

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as a LPN.
• Current State’s Driver’s License.
• One year experience as an LPN, home health preferred.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.
