Job summary

Angels Care Home Health is seeking an LPN Intake Coordinator to facilitate referral intake and coordinate care with the interdisciplinary team. You will manage the referral intake workflow using Home Care Home Base software to ensure smooth transitions.

The role supports LPN opportunities, with a Monday–Friday schedule and opportunities for professional development within a growing national provider.Benefits begin early, including health, dental, vision, and a 401(k) plan.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• One year experience as an LPN, home health preferred.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.

Responsibilities

• The primary function is to facilitate the intake referral process and coordinate care with interdisciplinary team and the referral source.
• Process all aspects of referral intake via Home Care Home Base software.
