Job summary

Angels Care Home Health in Cottonwood, AZ seeks an LPN Intake Coordinator to manage referral intake and coordinate care with the interdisciplinary team. You will facilitate referrals, document and track progress, and ensure smooth handoffs to clinical staff.

The role emphasizes collaboration, efficient intake processing, and effective communication with referral sources and patients, supporting quality home health services.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• One year experience as an LPN, home health preferred.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.

Responsibilities

• The primary function is to facilitate the intake referral process and coordinate care with interdisciplinary team and the referral source.
• Coordinate with referral source for timely processing and handoffs.
• Process all aspects of referral intake via Home Care Home Base software system.
