Job summary

Angels Care Home Health in Omaha, NE seeks an LPN Intake Coordinator to facilitate referral intake and coordinate care with the interdisciplinary team. The role uses Home Care Home Base to process all aspects of referral intake and ensure smooth transitions.

Applicants must have a current LPN license, a valid driver’s license, and at least one year of home health experience; schedule is Monday–Friday with comprehensive benefits and opportunities for growth.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as a LPN.
• Current State’s Driver’s License.
• One year experience as an LPN, home health preferred.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.
