Job summary

Angels Care Home Health in Grand Island, Nebraska is seeking a dedicated LPN Intake Coordinator to support the referral intake process and coordinate care with the interdisciplinary team. You will manage referrals via Home Care Home Base and liaise with referral sources to ensure timely, quality patient onboarding.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current state license as an LPN.
• Valid State Driver's License.
• One year experience as an LPN, home health preferred.
• Proof of hepatitis profile.
