Job summary
OneWell Health Care is seeking a dedicated Licensed Practical Nurse (LPN) in Philadelphia, PA to support individuals with IDD in home and community settings. You will implement care plans, administer medications, and promote health, safety, and dignity.
This field-based role reports to the RN Case Manager and requires PA licensure, CPR/First Aid, and proven nursing experience. You will document care in the electronic record and collaborate with families and the nursing team across
Qualifications
- Current and active LPN license in Pennsylvania.
- CPR and First Aid certification required.
- Completion of an accredited Practical Nursing program approved by the Pennsylvania State Board of Nursing.
Responsibilities
- Implement individualized care plans for IDD in home and community settings.
- Administer prescribed medications and treatments.
- Perform wound care, catheter care, tube management, and vital sign monitoring.
- Document meds, treatments, assessments, and incidents.
- Educate families and train staff; participate in case reviews.
