Job summary

OneWell Health Care is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing services in home and community settings across Pennsylvania. You will work under an RN Case Manager to implement care plans, administer medications, and support individuals with IDD and related needs.

Ideal candidates will have PA LPN licensure, CPR/First Aid, and at least 1 year of nursing experience, with preferred IDD/home health exposure.

Qualifications

• Current and active LPN license in Pennsylvania.
• CPR and First Aid certification (required).
• Completion of an accredited Practical Nursing program approved by PA Board of Nursing.
• Minimum of one year nursing experience.
• Experience supporting individuals with IDD, home health, or long-term care preferred.

Responsibilities

• Implement individualized care plans and administer medications.
• Perform clinical procedures including wound care and catheter care.
• Monitor and document patient condition; communicate changes to RN Case Manager.
• Provide education and training to families and caregivers.
• Maintain infection control and safety practices; participate in quality improvement.
