Job summary

InVision Human Services is seeking a Licensed Practical Nurse to coordinate medical care for people supported by InVision Customized Services. You will review medical histories, coordinate with physicians, and ensure timely treatment.

The role includes training staff on medical diagnoses and medications, participating in health-related goals, and advocating for the needs of individuals; travel within the service region and HIPAA compliance are required.

Qualifications

• Current PA Licensed Practical Nurse.
• Experience with Electronic Health Records (EHR).
• Experience with intellectually/developmentally disabled population a plus.
• Experience with medically and behaviorally complex cases a plus.

Responsibilities

• Review and maintain medical history and records related to diagnoses and care.
• Coordinate with physicians and providers to ensure treatment is received per regulations.
• Advocate for the needs and rights of people served when hospitalized or in treatment.
• Attend team meetings and health-related goal development and reporting.
