Job summary

Chapters Health is seeking a licensed Practical Nurse to provide hospice nursing care under RN supervision, focusing on patient comfort and family support. The role includes IV therapy, symptom management, and adherence to state regulations in Florida.

The ideal candidate holds an active LPN license in Florida, IV and BLS certifications, and a flexible schedule to meet patient needs. A valid driver’s license and reliable transportation are required for patient visits.

Qualifications

• High school diploma or GED required.
• Current LPN licensure in Florida.
• IV certification required by 11/10/2024; deadline 6/10/2025 if already employed before 11/10/2024.
• Active BLS for healthcare professionals from AHA or Red Cross.
• For Hope employees: BLS by 2025-01-01; for Capital Caring: by 2024-11-01.
• Valid driver's license and automobile insurance per policy.

Responsibilities

• Provide nursing tasks under RN supervision including continuous care, dressing changes, and catheter changes as per plan of care.
• Administer medications, manage symptoms, and support patient/family relationships according to policy.
• Observe patients for changes and report to RN; monitor and report adverse reactions.
• Document findings in the medical record per policy.
• Participate in emergencies and flexible staffing as needed.
• Cross-training to Hospice House roles where census requires.
