Job summary

Chapters Health is seeking a licensed practical nurse (LPN) in Florida. Under RN supervision, you will provide nursing interventions and patient/family education to maximize comfort and quality of life.

Responsibilities include wound care, IV/venipuncture, medication administration, and collaboration with the IDG to follow the care plan. A flexible schedule and strong communication skills are essential.

Qualifications

• High school diploma or GED or equivalent experience.
• Current LPN licensure in Florida.
• Active BLS for healthcare professionals from AHA or Red Cross.
• Florida-focused Alzheimer’s/Dementia training certification for PACE staff.
• BLS due by specified dates for current Hope/Capital Caring employees.
• Valid driver’s license and auto insurance per policy.
• Reliable transportation to meet visit schedule.
• Infusion Therapy course (30 hours) and in-house 8-hour class preferred.
• Clear written and spoken English communication.
• Flexibility to meet patient needs; safe practice within scope.

Responsibilities

• Under RN supervision, perform nursing interventions and patient education.
• Provide care events: dressings, catheter changes, and symptom management.
• Observe patient status and report changes to RN and physicians.
• Document care in medical records per policy.
• Observe for adverse reactions and report to RN.
• Perform venipuncture and IV-related tasks per procedure.
• Be available for emergencies and possible unit reassignment during disasters.
• Carry out additional duties as assigned.
