Empath Health is seeking an LPN for Hospice Field coverage in Venice, FL and Sarasota. The role supports patients with advanced illness under the RN case manager, providing bedside care, and collaborating with the care team to maintain comfort and dignity.

As a member of Empath Health, we help patients with advanced illness live meaningfully while supporting their families with warmth, expertise, and respect.** **Schedule:** Monday - Friday 8am to 5pm**Coverage Area:** Sarasota & Venice Under the direction of RN case manager, provides clinical, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition as needed.
