Empath Health's Tidewell Hospice is seeking an experienced LPN for Continuous Care in southwest Florida. You will provide bedside nursing, assist with care plans, and support families with compassion. Flexible 12-hour shifts and mileage reimbursement are offered.

Applicants must have Florida LPN licensure and at least 1 year of acute care; hospice experience is preferred. Competitive pay and benefits are included, with CEU support and growth opportunities.

What You’ll Do

• Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
• Continuous Care LPNs also assist Triage and Care Centers overnight when Continuous Care cases are pending.

What You’ll Need
