Job summary

Tidewell Hospice, part of Empath Health, offers compassionate, dignified care across southwest Florida. This 1:1 bedside nursing role covers the 8:00 PM–8:00 AM shift and requires two weekend nights per month, serving Manatee, Charlotte, DeSoto, and Sarasota counties with mileage reimbursement.

Under the RN case manager, you will provide continuous monitoring, support, and care aligned with the plan of care, while collaborating to maximize patient comfort and family support.

Qualifications

• Graduate of an approved nursing program and hold a current Florida LPN license.
• Minimum one year experience in an acute care setting; hospice LPN experience preferred.

Responsibilities

• Provide continuous bedside clinical observations, prescribed care, and physical/emotional support per plan of care.
• Report significant changes in patient condition and family needs to RN case manager.
• Assist triage and care centers overnight when Continuous Care cases are pending.

1:1 Patient Ratio | (8:00 PM – 8:00 AM), Must include 2 weekend nights per month

What You’ll Do

• Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
• Continuous Care LPNs also assist Triage and Care Centers overnight when Continuous Care cases are pending.

What You’ll Need

• Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
