Job summary

Empath Health's Tidewell Hospice in southwest Florida is seeking a dedicated LPN to join our patient care team. Working under an RN case manager, you will provide bedside clinical, physical, and emotional support to patients in the plan of care, and communicate changes to the team.

Requirements include graduation from an approved nursing program and a current Florida LPN license, plus at least one year in acute care and IV certification within six months of hire.

Qualifications

• Graduate of an approved nursing program.
• Current Florida LPN license.

Responsibilities

• Provide bedside clinical, physical, and emotional support to patients as directed in the plan of care under RN supervision.
• Process physician orders with RN to maximize patient comfort.

Schedule:

Monday - Friday 8am to 5pm

Under the direction of RN case manager, provides clinical, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition as needed. Provides support to family/caregiver reporting psychosocial/spiritual needs to team.

What You’ll Do:

• Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
• The LPN utilizes monitoring, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for the Patient/Family unit.

What You’ll Need:

• Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
• Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.
• Completion of an approved 30 hour IV certification course required within 6 months of hire
