Job summary

Brighton Hospice seeks a licensed practical nurse to provide home-based and facility-based care, joining an interdisciplinary team. You will perform psychosocial assessments and coordinate with community resources to support patients and families.

The role emphasizes visiting patients in their homes or facilities, delivering compassionate nursing care while maintaining compliance with state and federal rules.

Qualifications

• Current LPN licensure (in good standing) in the state of practice with one year of experience.
• Ability to work well within a team environment and collaborate daily with clinicians.
• Experience using an electronic medical records (EMR) system and understanding regulations.

Responsibilities

• Provide care and services for a group of patients, visiting them where they reside.
• Work in private homes, Skilled Nursing Facilities, and Assisted Living Facilities.
• Contribute to the interdisciplinary team with psychosocial assessments and interventions.
• Identify community resources to support patients and families.
