Job summary

Suncrest Hospice is seeking an LPN to provide care for a group of patients in home settings, including private homes and assisted living facilities. You will work as part of an interdisciplinary team, delivering psych-social assessments and linking patients with community resources.

You will support 1-1 patient care, collaborate with clinicians, and navigate electronic medical records while maintaining timely visits and accurate documentation.

Qualifications

• Current LPN licensure in good standing in the state(s) of practice with one year of experience.
• Ability to work well within a team environment with a team of clinicians.
• Ability to work one-on-one with patients and coordinate care with the care team.
• Ability to work independently, communicating via phone, email, and written notes and attending meetings.
• Time management to schedule visits and prioritize urgent needs.

Responsibilities

• Provide care and services for a group of patients visiting their residences or facilities.
• Collaborate with the interdisciplinary team on psych-social assessments and care planning.
• Learn and use the electronic medical records system.
• Drive as needed and maintain reliable transportation.

The Licensed Practical Nurse (LPN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and Assisted Living Facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.

• Current Licensed Practical Nurse (LPN) licensure (in good standing) in the state(s) of practice with one year of experience under license
• Ability to work well within a team environment to collaborate daily with a team of clinicians
• Work closely with patients on a 1-1 level but get strong support from a patient care team
• Can work independently and on your own from a day-to-day basis with activities which include phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of state and federal rules and regulations
• Time management skills to manage visits, documentation, meetings (i.e. to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
• Ability to learn an electronic medical records system
• Must be willing to drive and reliable transportation, valid driver’s license, and auto insurance
