Job summary

Brighton Hospice seeks a compassionate Registered Nurse to provide patient-centered care across home, SNF, and assisted living settings. You’ll join a multidisciplinary team, perform psychosocial assessments, and coordinate with physicians, social workers, and family members.

Responsibilities include case management, admissions, or resource roles, with emphasis on timely visits, accurate documentation in the EMR, and adherence to hospice regulations.

Qualifications

• Current RN licensure in good standing in the state(s) of practice.
• Ability to collaborate daily with multidisciplinary team members (nurses, social workers, chaplains, volunteers, and CNA leadership).
• Strong time-management and prioritization skills for visits, documentation, and meetings.
• Willingness to travel across the service area with reliable transportation and valid Driver's license.

Responsibilities

• Provide care and services for a group of patients in their homes or facilities (private homes, SNFs, assisted living).
• Work as part of an interdisciplinary team to address psychosocial assessments and community resource knowledge.
• Serve in Case Management, Admissions, or Resource role.
• Document care activities and coordinate with physicians and care teams within hospice regulations.
