Job summary

Maxim Healthcare in Richmond, VA is hiring a Licensed Practical Nurse to care for pediatric and/or adult patients in clients' homes. Starting pay is $30-$35/hour, negotiable by experience. Benefits include health, dental, vision, HSA, life insurance, PTO and a 401(k).

Responsibilities include planning and evaluating patient care, administering medications, and coordinating discharge planning with supervisors. Requires active LPN license and BLS, with health certificate and imaging on file.

Qualifications

• Active LPN license in the state of practice.
• Current Health Certificate as required.
• Current PPD or Chest X‑Ray.
• Current BLS certification.
• One year LPN experience preferred.

Responsibilities

• Uses the nursing process to plan, implement and evaluate care.
• Initiates plan of care and requests revisions as needed.
• Incorporates therapeutic, preventive and rehabilitative nursing actions.
• Responds to patient condition changes and informs physician or supervisor.
• Administers medications and treatments and documents effectiveness.
• Reviews care plans and coordinates education, discharge planning.

Responsibilities:

• Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
• Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change
• Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor
• Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention
• Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable

Requirements:

• Current Licensed Practical Nurse(LPN) License for the state in which the nurse practices.
• Current Health Certificate (per facility Licensed Practical Nurse(LPN) contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year prior Licensed Practical Nurse (LPN) experience preferred.
